MVA rehabilitation

What motor vehicle accident rehabilitation is

Motor vehicle accident rehabilitation uses assessment, hands-on treatment and exercise to help your body recover after a collision so you can return to work, driving and daily life. If you were injured, see a doctor first, then book a physiotherapy assessment.

Sprains, strains, bruising and other damage to muscles, tendons and ligaments (soft tissue injuries) are the most common injuries after a car accident. They can be painful and slow to settle, and some only show up days or weeks after the collision, which is why an early assessment is worth booking even when the accident seemed minor.

Why an early assessment helps

  • Hidden injuries can be found before they turn into long-standing pain.
  • A clear starting point is recorded for your recovery and your claim.
  • Treatment starts sooner, while the injury is still recent.
  • Billing is handled with your auto insurer directly.

Step by step

What to do after a car accident in Ontario, and how treatment gets covered

Ontario’s auto insurance system pays for assessment and treatment after a collision through accident benefits, regardless of who was at fault. The steps below are the ones that protect both your recovery and your claim. Physio Cottage handles the clinical forms and the billing; you focus on getting better.

1. Get checked, even if you feel fine

Whiplash, concussion symptoms and back pain often appear a day or two after a collision as adrenaline wears off. See a doctor or come for an assessment early, and tell them it was a motor vehicle accident so it is documented from the start.

2. Notify your insurer within seven days

Call your own auto insurer to report the collision and tell them you were injured. They open an accident benefits claim and send you the application package. If you were a passenger, pedestrian or cyclist without your own policy, the driver’s insurer or another policy may apply; we can help you work out which.

3. Return the application within 30 days

The application for accident benefits and the disability certificate, which a health professional completes, go back to the insurer. We complete the clinical sections and help you with the rest.

4. Treatment plan submitted by the clinic

After your assessment we submit a treatment and assessment plan to the insurer electronically. For most sprain, strain and whiplash injuries, treatment can begin right away under the Minor Injury Guideline without waiting for approval.

5. Direct billing, progress reporting

We bill the insurer directly for approved treatment and send progress updates. If your injury does not fit the minor injury category, we submit the evidence for a higher level of care and coordinate with your adjuster.

6. Recovery and discharge

Treatment continues until you have recovered or reached a plateau. We document outcomes clearly, which matters if you have a lawyer or a longer term claim.

Injuries we treat

Common injuries after a collision

Whiplash and neck pain

The most common injury after a rear end collision. Pain, stiffness and headaches that peak over the first few days. Early, gentle movement and graded exercise lead to better outcomes than a collar and rest; we start carefully and progress steadily.

Concussion symptoms

Headache, dizziness, fogginess, light sensitivity and poor sleep after a knock to the head or a hard jolt. After medical assessment, physiotherapy helps with the neck and vestibular components and with a paced return to work and screens.

Low back and mid back pain

From the bracing and twisting of impact, seat belt loading and sudden stops. Treatment restores movement, settles muscle guarding and rebuilds core and hip strength so sitting, driving and work become comfortable again.

Shoulder, chest and seat belt injuries

Bruising and strain along the seat belt line, shoulder injuries from bracing on the wheel, and rib soreness. These are assessed carefully for anything that needs imaging before rehabilitation begins.

Knee, hip and ankle injuries

Knees striking the dashboard, feet braced against the brake pedal, and pedestrian or cyclist impacts. Rehabilitation follows the same staged approach as sports injuries.

Persistent pain and anxiety about driving

Some patients develop lingering pain and a real fear of getting back in a car. We treat the physical side with a graded program and coordinate with your doctor for support with the rest.

Treatment

What motor vehicle accident rehabilitation involves at Physio Cottage

  • Thorough first assessment: a full history of the collision and your symptoms, screening for red flags, and baseline measures that are documented for your claim.
  • Early, gentle care: manual therapy, soft tissue release and reassurance based education in the first weeks, with movement started early rather than prolonged rest.
  • Progressive exercise: mobility, then strength and endurance, planned toward your real goals: driving without pain, a full shift at work, lifting your children, returning to sport.
  • Multidisciplinary care in one place: physiotherapy, chiropractic care, registered massage therapy and acupuncture under one treatment plan and one chart, which also keeps your claim paperwork simpler.
  • Clear documentation: progress is measured and recorded at each stage, which supports your accident benefits claim and any legal process.

Working with lawyers and insurers

Many of our patients have a personal injury lawyer, and many do not. Either way, we work the same: we treat the injury, report accurately and respond promptly to requests from your adjuster or your lawyer’s office. We do not inflate findings and we do not minimize them. If you need copies of your records for your claim, ask and we will provide them according to privacy rules.

If your insurer denies a treatment plan, there is a dispute process, and we will explain your options and provide the clinical information you need.

Good to know

Facts about accident benefits that surprise most patients

  • Fault does not matter for treatment. Accident benefits come from your own insurer whether or not the collision was your fault.
  • Passengers, pedestrians and cyclists are covered. If you do not own a car, a policy in your household or the driver’s policy generally applies.
  • You choose your clinic. An insurer can suggest providers; the choice of where you are treated is yours.
  • Extended health benefits are used first for some items. If you have a workplace plan, the insurer may require it to be billed first for certain services. We handle that coordination.
  • Timelines matter. Seven days to notify, thirty days to return the application. Late is not fatal to a claim, but it makes everything harder.
  • Treatment is time limited. Benefits for minor injuries are capped, so starting early and attending consistently gets the most from them.

Practical questions

Questions we hear after a collision

Do I pay anything for treatment after a car accident?

For approved accident benefits treatment we bill your insurer directly. Where the insurer requires your extended health plan to be billed first, we direct bill that too, so out of pocket costs are usually nothing or minimal.

How soon should I start treatment?

Within the first week if possible. Early assessment documents your injuries and early movement improves recovery. Same day and next day assessments are usually available.

I did not go to the hospital. Is that a problem?

No. Many people with soft tissue injuries do not need emergency care. What matters is that a health professional assesses and documents the injury soon after the collision; we can be that professional.

My symptoms started two days after the accident. Is that normal?

Yes. Delayed onset of neck, back and headache symptoms is very common after a collision. Report them to your insurer and book an assessment.

Can I have treatment if the other driver was uninsured or fled?

Yes. Your own policy, a household policy or Ontario’s fund for uninsured situations may apply. Report it to the police and your insurer, and we will help with the treatment side.

How long does recovery take?

Most whiplash and soft tissue injuries improve substantially within six to twelve weeks of consistent treatment and home exercise. Some take longer, especially with concussion symptoms or pre existing conditions, and we adjust the plan accordingly.

Do I need a lawyer?

That is your decision and depends on the severity of the collision and your injuries. We are happy to work with your lawyer if you have one, and our treatment does not change either way.

Whiplash recovery

Whiplash after a collision: what recovery usually looks like

Whiplash is the most common injury we treat after a motor vehicle accident, and it is also the most misunderstood. The neck is whipped forward and back in a fraction of a second, straining muscles, ligaments and the small joints of the spine. Most whiplash injuries are classified as mild to moderate, and most recover well with early, active treatment. Here is the pattern we typically see:

Days 1 to 3

Stiffness and pain build as inflammation peaks, often with headache, trouble turning the head and sleeping. Gentle movement within comfort, heat or ice for comfort, and an early assessment to document the injury and rule out anything serious.

Weeks 1 to 3

Manual therapy to restore movement, gentle range of motion and posture exercises, and reassurance. Returning to normal activities, including work and driving where possible, is encouraged; prolonged rest and soft collars are associated with slower recovery.

Weeks 3 to 8

Strengthening the deep neck and shoulder blade muscles, treating any headache and dizziness components, and building tolerance for sitting, screens and driving. Most patients are substantially better by the end of this phase.

Beyond 8 weeks

A minority have lingering symptoms, often linked to higher initial pain, previous neck problems, concussion symptoms or stress about the collision. These patients need a paced, graded program and sometimes support from their doctor; we recognize this early and adjust rather than repeating the same treatment.

Concussion and the neck

When a collision affects your head as well as your neck

You do not have to hit your head to sustain a concussion in a collision; the same whipping motion can do it. Headache, fogginess, dizziness, sensitivity to light and screens, poor sleep and irritability in the days after an accident should be reported to your doctor. Once a concussion has been diagnosed, physiotherapy has a real role: the neck is often a major contributor to post concussion headache and dizziness, and vestibular exercises help the balance system recover. We also guide a paced return to screens, work and driving, which is the part most patients find hardest to manage alone.

If you develop worsening headache, repeated vomiting, confusion, slurred speech, weakness or a seizure after a collision, go to an emergency department immediately.

Returning to driving, work and sport

  • Driving: you need to turn your head freely to check mirrors and blind spots. We work on rotation early and tell you honestly when you are ready; short daytime drives first.
  • Desk work: often possible within days with breaks and a reasonable screen setup; we provide a note on modified hours or duties if needed.
  • Physical work: staged return with lifting and overhead limits that lift as strength returns. We complete the forms your employer asks for.
  • Sport and gym: once range of motion and strength are back, usually after the first month for mild injuries, progressed like any sports injury.

Special situations

Pre existing conditions, older adults and children after a collision

If you had back or neck problems before

A collision can aggravate an old disc problem, arthritis or a previous whiplash. Treatment is still covered; the plan accounts for your baseline and we document carefully what changed because of the accident.

Older adults

Recovery can take longer and screening for fractures is more important, especially with osteoporosis. Treatment is gentler at first and progresses steadily; the goal of independence at home is central to the plan.

Children and teenagers

Children are often in the back seat and can have neck pain, headaches and anxiety after a collision. We assess them with a parent present and keep treatment gentle and short.

Pedestrians and cyclists

Injuries are often more severe and more varied, from fractures to road rash to concussion. Rehabilitation begins once the acute medical care is complete, and accident benefits still apply.

Anxiety and sleep problems

Very common after a collision and a major factor in slow recovery. We address what we can through movement, pacing and sleep advice, and we encourage you to involve your doctor for the rest.

Injuries that appear weeks later

Some pain, especially in the low back and shoulders, shows up as you resume activity. Report it to your insurer and your clinic; late appearing symptoms are common and can be added to the treatment plan.

More answers

Further questions about treatment after a car accident

What is the Minor Injury Guideline?

It is the part of Ontario’s accident benefits system that covers sprains, strains, whiplash and similar injuries with a set amount of funding and a streamlined approval process, so treatment can begin quickly. Most collision injuries fall under it. If your injury is more serious, or you have a pre existing condition that affects recovery, we submit the evidence for care outside the guideline.

What forms will I need to sign?

The application for accident benefits, a disability certificate completed by a health professional and the treatment plan prepared by the clinic. We complete the clinical portions and explain the rest; most patients spend less than twenty minutes on paperwork in total.

Can I switch to Physio Cottage if I started treatment elsewhere?

Yes. Ask the previous clinic to send your records, tell your adjuster you are changing providers, and we take over the treatment plan from where it stands.

What if I was partly at fault?

Accident benefits are paid regardless of fault. Fault affects a separate claim for damages, not your right to treatment.

How long after the accident can I claim treatment?

Notify your insurer as soon as possible; the timelines are short. If you missed them, claims can still proceed with a reasonable explanation, so do not give up. Book an assessment and we will help you sort out the process.

Will treatment affect my insurance premiums?

Accident benefits claims for your own treatment are not the same as at fault claims. Questions about premiums are for your broker, but injured people should not avoid treatment out of fear of premiums.

Do you provide reports for my lawyer?

Yes, with your written consent. We provide clinical notes and reports according to privacy rules, and we respond promptly.

Can I combine physiotherapy, chiropractic care and massage under one plan?

Yes. Multidisciplinary plans are common after collisions, and because all three are in one clinic we coordinate the plan, the scheduling and the billing.

Glossary

Accident benefits terms explained in plain language

Accident benefits

The part of every Ontario auto insurance policy that pays for medical and rehabilitation care, income replacement and other costs after a collision, regardless of fault.

Adjuster

The insurance company employee who manages your accident benefits claim, approves treatment plans and handles payment. We communicate with them on your behalf for treatment matters.

Application for accident benefits

The form that opens your claim with your insurer. It must be returned within the stated deadline; we help you complete it.

Disability certificate

A form completed by a health professional describing your injuries and how they affect your ability to work and function. Needed for income replacement and other benefits.

Treatment and assessment plan

The clinic’s proposal to the insurer for the care you need, submitted electronically. Treatment under the Minor Injury Guideline can begin without waiting for approval.

Minor Injury Guideline

The streamlined pathway for sprains, strains and whiplash, with set funding and faster access to care. Most collision injuries are treated under it.

Pre existing condition

A health problem you had before the collision. It does not disqualify you from benefits, and if it makes recovery harder it can support care beyond the minor injury pathway.

Independent examination

An assessment the insurer may arrange with another practitioner to review your treatment. Attend it; we provide your records and respond to the findings.

Tort claim

A separate legal claim against an at fault driver for losses not covered by accident benefits. Handled by a lawyer, not by the clinic; our records support it.

Serving collision patients across Scarborough and east Toronto

Physio Cottage is at Victoria Park and Ellesmere, minutes from Highway 401, the Don Valley Parkway and the busy corridors of Lawrence, Eglinton, Victoria Park and Kennedy where many of the collisions our patients describe happen. We see patients from Wexford, Agincourt, Parkwoods, Don Mills, East York, North York, Markham and Pickering. Evening and Saturday hours mean you can start treatment without missing work, and on-site parking matters when getting in and out of a car is still painful. Bring your claim number if you have one, your health card and insurance details, and any hospital or doctor’s notes, and we handle the rest.

A typical course

What recovery after a collision looks like in practice

To make the process concrete, here is a typical example drawn from the kind of case we treat every week, with details changed. A patient in her forties is rear ended at a red light on Lawrence Avenue. She feels shaken but fine, drives home, and wakes the next morning unable to turn her head, with a headache behind the eyes. She calls her insurer that day to report the collision and the injury, and books an assessment with us for the following afternoon. At the first visit we take a full history, screen for anything that needs a doctor or imaging, measure her neck range and strength, document the findings for her claim and complete the clinical forms. She leaves with gentle movement exercises, advice on sleep positions and heat, and a plan for twice weekly visits. The treatment plan is submitted to the insurer the same day under the minor injury pathway, so care begins immediately.

Over the first three weeks the focus is manual therapy to restore movement, settling the headaches through treatment of the upper neck, and getting her back to desk work with a note recommending screen breaks and a short modified schedule. By week four she is driving comfortably and the headaches are infrequent. Weeks four to eight shift to strengthening the deep neck and shoulder blade muscles and building tolerance for the long drives her job involves. Registered massage therapy is added on alternate weeks for the shoulder tension that is slowing her exercise progress, billed under the same plan. At week ten she is discharged with a short maintenance routine and a note to call if anything flares. Her insurer received progress reports throughout, she paid nothing out of pocket, and her records are complete should she ever need them. Not every case is this smooth, and we say so when it is not; but this is the shape most whiplash recoveries take when treatment starts early and the patient does the home program.
